Forms and Applications
Most of the forms and applications required for our programs will be the responsibility of the lender. However, there are two that you, the real estate agent, should be aware of:
013-Seller Affidavit
All sellers, whether individuals or corporations, must complete and sign this form, which includes occupancy information, acquisition cost information, and seller certifications. A Power of Attorney cannot be used in conjunction with this form.
022-Addendum to the Sales Contract
This form is required only when the buyer is requesting a Downpayment Assistance Program second mortgage.
The buyer and all sellers must sign this form.

for a 30-year, fixed-rate mortgage (90-day lock in) without NCHFA downpayment assistance.
